I refer to your request of 16 August 2021 to the Department of Health (department)
seeking access under the
Freedom of Information Act 1982 (FOI Act) to:
The document I seek is a copy of the consent form that people being injected with
so-called COVID-19 “vaccines” such as the “Pfizer jab” are required to sign before
being injected with a substance that only has “provisional approval” from the TGA.
Decision
I am authorised under subsection 23(1) of the FOI Act to make decisions in relation to
Freedom of Information requests. I am writing to notify you of my decision on your
request.
I have identified 1 document falling within the terms of your request being the
COVID-19 vaccination consent form for COVID-19 vaccination. It is publicly

Internal review
Under section 54 of the Act, you may apply for internal review of this decision.

Australian Information Commissioner (OAIC) for review of my decision by the
Information Commissioner (IC).
