I am lodging a request for documents pursuant the Freedom of Information Act 1982.

The National Australia Bank has been the subject of media attention recently.

I have previously provided the ACC with a copy of a document produced by NAB Wealth. This document is titled "Carlton & United Breweries Superannuation Plan Participation Schedule".

This document is a "cut & paste" document from earlier documents.

The document I seek is a copy of a document that describes why this document has been sourced from earlier documents that bear the signatures of Ken Jarrett and Geoffrey Cohen.

The document includes the words "NAB Wealth Scandal" in its title.
The words "The Cohen Resolution" and "The Jarrett Deed" appear in the body of this document.

The Power of Amendment - Regulation 50 is included in the Appendices.

The search period is from 1 March 2015 to the present.
