Dear NSW Department of Education,

I would like to make a request for information under Government Information Public Access Act 2009.

I am the mother of Cody Ian List 17/05/08 who is a student at Kirrawee Public School.

On Wednesday 29 August 2018 as serious incident occurred between Cody List and another male student in the classroom of 3/4w while the teacher was present. It resulted in my son being punched in the face and receiving a bleeding nose.

On Thursday 30 August 2018 my husband and myself attended a meeting with the following parties at the school:-
Principal: Selwyn Williams.
Assistance Principal: Patrick Morgan
3/4w Teacher: Sarah Whicker
Meeting time: 3.30 pm to 4.00 pm

Patrick Morgan was taking handwritten minutes of the meeting in a black note book. At the end of the meeting I asked for a FULL copy of minutes - there was approximately 4 pages of written notes. I was told that he would type them up and give them to me. To date I have not received a full copy of the minutes.

I request the following information:-

1. Copy of the incident report dated 29 August 2018 for Cody List.
2. Full copy of hand written notes taken by Patrick Morgan during the meeting on 30 August 2018.

I would appreciate a response within the 20 working day legislative timeframe.
