In order to submit a valid request for access to records under the Freedom of Information Act 1992 (FOI Act), the following are statutory requirements:

1. An Australian address (to which notices under the FOI Act can be sent).
2. Payment of the $30.00 FOI application fee (noting this can be made via an online payment, however an invoice will need to be raised by the Department of Health and emailed to you for this purpose).
