I am authorised to respond to your request for access to documents under
the Freedom of Information Act 1982 (the Act) received by this office on
20 January 2023, in which you sought access to:

 

A complete copy of the Medical Records your late Mother, Heather Grimshaw.

 

 

S.17 of the Freedom of Information Act sets out the requirements for a
valid request. In order to be valid, a request must:

 

·         be in writing;

·         have the appropriate FOI application fee of $30.60 attached -
The application fee is a fixed cost which is non-refundable. The only
exception is for eligible applicants who are in receipt of a benefits
card, in which case the hospital will waive this fee. (copy of card must
be provided with application)

·         provide at least one form of appropriate ID, ie driver’s
license, passport, birth certificate

·         provide written authority from the person who is the subject of
the information or in this case, the next-of-kin or executor of the
deceased.
