Name of Police Officer responsible for issuing Penalty Notice 4042037717 on 2/12/17

George made this Government Information (Public Access) request to NSW Police Force

This request has been closed to new correspondence from the public body. Contact us if you think it ought be re-opened.

Waiting for an internal review by NSW Police Force of their handling of this request.

Dear NSW Police Force,
I received a parking fine on 2nd December 2017 for parking in a disabled park . Penalty Notice 4042037717. Because I was suffering a life threatening illness and disabled following an accident time I explained my predicament to the Policeman I consider his action both irresponsible and reprehensible and that he should either be reprimanded or counselled on better policing. I wish to know his name so I can take up the matter with his commanding officer at Wagga.

Yours faithfully,

George Molineux

Hello George,

In order to make a request for NSW Police Force information, an applicant
is required to submit a completed 'Formal Access' application under the
Government Information (Public Access) Act 2009.

Please find attached the ‘Formal Access’ application form for completion.
You should follow all instructions on the form, include as much
information and guidance as possible about the information you require and
sign the form before submitting it.

When seeking access to personal information, an applicant must provide a
certified copy of their proof of identity. Certified means that the proof
of identity must be verified (i.e. signed and dated) by an authorised
person (Justice of the Peace, Doctor, Pharmacist, Legal Practitioner,
Postmaster – Australia Post).

Acceptable forms of proof of identity include any one of the following: -
an Australian Driver’s Licence including a photograph, signature and
current address; a current Australian passport or other proof of signature
and current address details (such as a Government issued identity card or
Government issued proof of age card).

Each ‘Formal Access’ application submitted must be accompanied by an
application fee of $30.00. This fee can be paid by credit card, cheque or
money order.  Cheques and money orders should be made payable to ‘NSW
Police’.

If your application form is submitted via email, we can only accept a
credit card authority form. Alternatively, if you submit your application
through the post, all payment methods can be accepted.

If you are applying for someone else's information, please be aware that
it is unlikely to be released to you unless you have obtained a dated and
signed authority, authorising specifically the release of that particular
information.

The NSW Police Force has up to twenty (20) working days under the Act
(subject to any extension allowed for under the Act) to deal with an
application.  If any extension of time is required to deal with an
application, we will let the applicant know in writing.

Please e-mail the completed forms (formal access application and credit
card authority form) to the NSW Police Force External Information Access
Unit (EIAU) at: [NSW Police Force request email].

The Officer would be in a public place discharging a public office. Unless the Officer was 'undercover', there should be no issue in supplying requested information. Joys of public life

Dear #GIPAAPP,
A response to our FOI request is long overdue. By law, your office is required to provide information as per our request. It seems a little hypocritical that the police issue a fine to the letter of the law (without common sense to a person in a medical emergency) but don’t follow the law when it suits.

Could you please attend to this request ASAP. We have waited, patiently, long enough.

Yours sincerely,

George
