NDIS - "Risky Hires" and Non-Australia Citizens (Executive Positions)

Dear National Disability Insurance Agency,

Please provide a copy of the 'risky hire' decision made to engage Mr Oliver Bladek.

Mr Bladek recently, publicly declared his hiring into the NDIS was a 'risk'.

Pease provide a copy of the risk assessment and waiver informing this NDIA senior executive hire.

Ironically, Mr Bladek seems to have been engaged as the Chief Risk Officer at the NDIA, with no past or specific experience in this area, before moving around to what appears to be a total of 3 titles in as many years, according to his own public declarations.

Can you also confirm he was an Australian citizen during his time with the NDIA?

Was any of his appointments (x3) a security designated position? If so, can you confirm, he was fully vetted and held a valid Australian security clearance during his tenure at the NDIA?

Especially since it appears that funding increased nearly 100% (AUD$ 13billion to AUD$29 billion) along with participants increasing nearly 100% (310,000 to 500,000+) during his time engaged at the NDIA.
