Thank you for your below request received by the Chief Minister, Treasury & Economic Development Directorate on 10 January 2022. Part of the information you are seeking (Information from CYPS relating to medical treating professionals, dates, and medication if given, along with diagnoses) is held by the Community Services Directorate and accordingly has been transferred to that Directorate effective 10 January 2022.

In regard to the below two points you will need to make an application for this information through the Australian Federal Police.

1. Information obtained by SACAT act policing team including incident report and photographs taken of myself.

2. Information from CYPS relating to medical treating professionals, dates, and medication if given, along with diagnoses.

Good afternoon Ms Murdoch,

 

The Community Services Directorate have received this request via Access
Canberra and part of it will be progressed by this FO Team.

Other parts are likely to be referred to ACT Health and the AFP for
consideration and direct response back to you.

 

The part of the request that CSD will respond to is:

“Information from CYPS relating to medical treating professionals, dates,
and medication if given, along with diagnoses.”

 

As you are specifically seeking medical records, this request will be
processed under the Health Records (Privacy & Access) Act 1997 as the
Freedom of Information Act 2016 does not allow for the provision of
medical records.

At this time, your application is not considered valid.  Requests require
a copy of personal identification (eg Driver licence, Photo Pension Card,
Proof of Age card, etc) as proof of identity. May I ask you to provide
this to us please?

On receipt, your application will be fully registered and considered valid
at that point.

To assist in processing your request, could you please clarify the time
period related to your request please? Are you seeking any medical records
on any CYPS files from the beginning of your involvement with that Service
or only for a specific time period, eg relating to any
incidents/approximate dates?

When you have confirmed this and provided your ID, the time allowed to
respond under the Health Records (Privacy & Access) Act 1997 is 10 working
days, however depending on the response to searches, the volume of
information identified we may need to request additional time to allow us
to ensure a full and accurate response is provided to you.  If this is the
case, we will contact you. Do you also have another direct email address
that we may contact you on?
