Please call the hospital’s contact number as provided above and ask for
the medical records department. Their officers will be able to assist you
with the process for applying for access to relevant records. Generally
requests for hospital health records are processed under the Health
Records and Information Privacy Act 2002:

application fee of $33 plus photocopying fees. Concession holders may be
eligible for discounted fees.
