Good Morning

 

To obtain this information you will need to complete a Freedom of
Information application.  This can be done as follows:

 

1.     Complete the attached form, or

2.     Complete an online application at

3.     Attach payment of $37.50 – online payments can be made by
credit/debit card.

4.     If you have a valid concession card of any type attach a copy of
that to the application and then the application fee is waived.  A valid
full time student ID also provides for a concession.

5.     Attach one form of photo ID.

6.     If you are applying on behalf of someone else, please provide a
signed, written authority from that person stating you can have access to
their information.

7.     Clearly state the documents you require including, to the best of
your knowledge:

·       Identify the document type/s

·       Type of incident

·       Time/Date/Location of incident

·       Name(s) of persons involved

·       Report number (if known)

 

NB: Freedom of Information have 30 days to deal with applications although
in some cases where it will take more time you will be contacted about any
extra time required.
