Thank-you for your request of 18 December 2021, in which you have sought
access to information under the Freedom of Information Act 1982 (Cth)

Based on the information provided, it appears that you are seeking to
access:

 

“Documents outlining the number of times Victoria Police contacted Loretta
Indrigo while Izabella Zarafa was listed as missing”

and Izabella Zarafa’s identity to process your request. Documents you can
provide for this purpose include:

•             Australian driver licence;

•             Passport;

•             Birth certificate;

•             Australian citizenship certificate;

We require identification document(s) that are certified by a person
having the power to sign a Commonwealth statutory declaration. A list of
such persons is on the Commonwealth Attorney-General Department's website.

 

To assist me with your request for access to documents, please provide
certified copies of one of the above documents for both yourself and
Izabella Zarafa by 5 January 2021. As outlined above, if you do not
respond by this date I will assume you do not wish to proceed and treat
your request as withdrawn.
