INSTRUMENT OF AUTHORISATION 
 
I, GLENYS BEAUCHAMP, Secretary of the Department of Health and principal officer for the purposes 
of the Freedom of Information Act 1982 (the Act), acting under subsection 23(1) of the Act, now: 
(1)  REVOKE all previous instruments of authorisation approving arrangements made under the Act; 
and 
(2)  APPROVE the arrangements set out in Schedule 1 and 2, that is, that each person from time to 
time holding, occupying or performing the duties of an officer or class of office described by the 
classification specified in part 2 of a Schedule to this instrument, is authorised to make decisions 
as specified in part 1 of the Schedule.
