Annexure A – Declared Conflicts and Management
*A members interests include those of an associate, close relative or a company or other body that you
may be a member of. To comply with the
Privacy and Personal Information Act 1998, when providing
information about associated persons, do not identify the person, you need only identify your relationship
with that person and the interest.
Description of conflict: Please provide a brief outline of the nature of the conflict, including the details of
the organisation or relationship concerned. Where possible please also include any information relating to
how you expect your role or responsibilities may be affected by the conflict.

• An interest may emerge, financial or otherwise, in the Western Parkland City or in relation to
the decisions made by the Commission
Western Sydney Community Forum Inc
• Employed as the Chief Executive Officer
• A policy interest may emerge in relation to decisions made by the Commission
• From time to time, the Commission may extend an invitation to Western Sydney Community
Forum for advisory, representative or consultative purposes
Management strategies: Please list any actions to resolve, avoid or manage identified conflicts of interest
above. Where appropriate you may seek advice about appropriate management strategies from the Chief
Commissioner or Chief Executive Officer prior to completion of this declaration.
South Western Sydney Local Health District
• Not be present at or take part in deliberations of the Commission about that matter
• Declare a conflict of interest at the Board of South Western Sydney Local Health District and
not represent the Commission in any related deliberations
Western Sydney Community Forum Inc
• All Commission related matters to be delegated to alternate senior officer of the organisation
• Declare a conflict of interest at Western Sydney Community Forum Inc and not represent the
Commission in any related deliberations
