Description: Client s. 47F(1)
 attended church at s. 47E(d)  as part of a weekly visit to this 
church. He went with 8 other clients who regularly attend this church.
Escort left IRH with 9 clients on board around 9:48am arriving at the church 9:55am. All clients 
made way into the church awaiting start of the Mass. The service commenced around 10:00am and 
the two escort officers covered the exits from the outside.
The service finished around 11:00am and when the clients made there way out of one of the doors 
and the second officer making his way to that location through the church. It was discovered one 
of the clients was missing. A search of the immediate area was undertaken by one of the officers 
whilst the other officer kept watch on the other clients. 
Phone contact was made with the T&E supervisor and staff where dispatched to the site to assist 
with the search. 
Serco On call managers. 22(1)(a)(ii)  was contacted at 1115am and Diac on call Manager s. 22(1)(a)(ii)
s. 22(1)(a)(ii)  was contacted at 11:40am.
NSW Police where contacted by CSW s. 22(1)(a)(ii)
 at 11:45am, he spoke with Constable

DIAC On call s. 22(1)(a)(ii)
 was contacted by T&E Manager s. 22(1)(a)(ii)
 at 2:39pm to 
confirm type of incident.
