In response to your informal application lodged under Government
Information (Public Access) Act 2009 (GIPAA), regarding information
related to food outlets in the Council, please be advised that required
information is available under formal access to Information and in
statistical manner only (with no names, addresses etc.), which means that
a formal application should be lodged (form that has to be filled up is on
the web site) and accompanied with a $30.00 payment to make the
application valid under Cl 41. Our preliminary consultation with the
health control staff had shown that the information for three years long
period is not available from just one source and some time has to be spent
to collect it and process will be (for two former Councils) much longer
than one hour ($ 30.00 paid when lodged) and additional processing fee
should be certainly expected if a formal one is received in this case.

Dear Svemir Popovic,

This request is very simple as all the information I am asking for is contained in an annual report all councils provide to the NSW food Authority. The food authority send an email form each year and it is emailed back to them. Lane Cove council had it in their document management system but if you do not you can request it from the food authority and they will email it back to you.

The Food authority will not release the forms as it is third party information so I have to request it from all councils individually. I have already received this information from several councils without the need for a GIPA or any paper work as it is an informal request.

related to food outlets in the Council, after your clarification made to
our request for a formal application to be lodged, please be advised that
our relevant officers, following your instructions that you are after
information from the Annual Reports submitted to the State Authorities,
prepared bellow list as an answer to your request. Due to the Councils
amalgamation, which currently taking place, information is related to the
former Hurstville and Kogarah Councils, which operate as Georges River

Are the fines both the Formal Improvement notices and the PINs (Name and shame) or are they separate, if so can I please get the number for each of these in each year?
