2. Of those waiting patients as of 14 December waiting for a trans-oesophageal echocardiogram can the wait times be broken down by LHD.
3. Of those waiting patients as of 14 December waiting for a trans-oesophageal echocardiogram can the numbers be broken down by:
- less than 6 months
- 6-12 months
- 12-18 months

in order for searches to commence to confirm relevant information is held
by the Ministry of Health.
Please note that all formal GIPA applications in NSW require the $30
application fee to be considered valid under the GIPA Act. The cost for
processing is then $30 per hour for non-personal requests.
If you wish to submit an application, please complete the attached GIPA
application form and submit it with payment for the $30 GIPA application
fee. Guidance on how to submit your application and make payment is
available on the attached application form, and further guidance on the

ensure the scope of your request is clearly defined.
