Victorian Department of Health

Dear Mr Smith

Freedom of Information Application - F17/1094

I refer to your application under the Freedom of Information Act 1982
requesting access to:

"I request documents related to the ten most recent instances where an
employee has sought information, advice, guidance or opinion on their
social media use in a private capacity.

I limit the scope of documents to:

- the original request from the employee
- the agency/department's response
- any follow-up questions and response
- only those sent to a relevant HR / conduct / social media (or similar)
team (rather than managers across all areas of the organisation)
- where the original request was created in the last 2 years"

Your application was received on 8 August 2017.

The Act provides that an application fee of $28.40 must accompany each
request. Please forward a cheque or money order for $28.40, made payable
to the Department of Health and Human Services at GPO Box 4057, Melbourne,
Victoria 3001.

The Act also provides that the application fee may be waived or reduced if
payment of the fee would cause financial hardship. If you consider this to
be the case the fee may be waived or reduced on production of a statement
of how the payment of the application fee would cause you hardship. I have
included a copy of the statement of hardship form that you need to
complete if you would like the application fee waived.

Please note your request cannot proceed until the application fee or your
statement of hardship has been received as it is not a valid request. I
would appreciate it if you would respond by Tuesday 5 September 2017,
otherwise I will assume that you do not wish to proceed with your request
and no further action will be taken by the department.

I can be contacted via email at [Victorian Department of Health and Human Services request email] or on (03) 9096 2004.
Please quote our reference number (F17/1094) in any correspondence.
