Dear NSW Ausgrid,

The following documents forming the basis of this request pertain to Hopewell Street Paddington NSW 2021 (the area/the subject area) from 1 January 2020 to the present day:

1. Documents in relation to the lighting of the subject area including alterations, maintenance, repairs and renovations carried out to the said area by Ausgrid, employees of Woollahra Council, contractors, subcontractors or any other person or agent.
2. All documents including investigation reports, witness statements, incident reports, claim reports, claim forms, injury reports, accident reports and any related correspondence by way of letter, e-mail or any form of correspondence concerning any accidents or injuries on Hopewell Street Paddington NSW 2021.
3. Any document relating to complaints (including phone calls, e-mails, text message and any form of communication), investigations, reports concerning the general condition and safety of the abovenamed street including lighting of the area.
4. All documents including job requisitions, job orders, invoices, reports and summaries relating to the repair, maintenance and general upkeep of the area
5. All documents including plans, engineer reports, inspection reports, record books, all development or other applications, approvals and consents to development or other applications relating to the construction, renovation, alteration, maintenance and repair of that area.
6. All documents showing or tending to show any alterations, maintenance, repairs and renovations carried out to the said area by Ausgrid, employees of Woollahra Council, contractors, subcontractors or any other person or agent.
7. All documents detailing copies of any contracts with contractors and/or subcontractors for works regarding lighting
8. CCTV footage of the subject area from 2 January 2024 to 16 January 2024.
9. Copies of documents relating to the polices and systems in place for the lighting of the subject area and any changes to lighting.
10. Light Maintenance Schedules
