scope of your request, I wish to advise that the Department of Health does
not hold these records.

To access the relevant documentation, you will need to apply directly to
the hospital where you received your treatment. Each hospital manages its
own records and processes Freedom of Information (FOI) applications

If your records are held by a private practice/hospital, you will need to
contact them directly to request access.

In light of this information, no further action will be taken in relation
to this FOI request.
