How many bus routes have had Opal card readers malfunction?

For how long on average is the card reader inoperative on a bus and which routes were affected?

In total how many times has an Opal card reader malfunctioned on a bus since the readers were rolled out and how many times this year?

What is the approximate loss of revenue for these malfunctions?
