I would like to request a record for myself holding a Queensland drivers licence. I can provide my personal details when required, I believe I held a license from 2004 - 2016 excluding some disqualification time. I require this information to help me obtain a Canadian drivers license.

Thank you for your enquiry received at the Department of Transport and
Main Roads (TMR). 

 

To enable your request to be processed we require you to return the
following:

 

·         Complete the enclosed “Release of Information Application” form.

 

·         Provide certified copies of your identification. If you are
unable to provide a Queensland issued driver licence, industry authority,
marine licence indicator or adult proof of age card you will need to
provide 3 evidence of identity documents. Please do not send original
identification. Ensure banking card numbers are not visible. The copy must
be certified by an approved witness and contain the words “I have sighted
the original document and certify this to be a true copy of the original’.
An approved witness (or local equivalent) may be a:

• Doctor

• Police Officer

• Consular or ambassadorial officer

• Solicitor, barrister or judge

• Justice of the Peace or a Commissioner for Declarations

• Notary Public or a person authorised by law to witness and sign
declarations

 

The witness must sign and print their full name, position
title/designation and contact details (including office stamp).

 

You may have to pay a fee to have the documents witnessed. All documents
are to be witnessed in English or have an English translation.

 

·         Pay the applicable fee of $24.05 per search by cheque or money
order payable on an Australian Bank and made payable to Department of
Transport and Main Roads.  Please note that for each box you select at
Question 4 (a) on the Release of Information Application form, a fee of
$24.05 is payable. 

 

To enable processing of your request please forward the required
documentation to the following address:

·         Alternatively,  you may be able to arrange for a friend or
relative to pay the applicable fee on your behalf at any QLD TMR Customer
Service Centre, where it can be paid by cash, card or cheque. 

Please ensure they have your full name, date of birth and driver licence
number and then email confirmation of payment to:
