Dear Ms Waterhouse,
I refer to your request “to access the application made by the City of
Joondalup for an upgrade to the BMX track in Shepherds Bush Park in
Kingsley”. I understand that you have spoken with a senior Lotterywest
Grants Manager about this grant, but would like to proceed with an FOI
application.
In order to submit a valid FOI application under the Freedom of
Information Act 1992 (WA) you must
· provide, in writing, a description of the documents you want
(This requirement has been met already by your previous email. However, it
may be necessary for us to contact you further if more precise detail is
required – for this purpose, providing your telephone number would also be
useful),
· give an Australian address where notices can be sent (not an
email address)
· pay an application fee of $30 – this can be done
by sending a cheque made out to Lotterywest to:
§ Freedom of Information Coordinator
Lotterywest,
Locked Bag 66,
Subiaco, WA, 6904
or by electronic transfer to:
Account Name: The Lotteries Commission of
WA
BSB: 066-040
Account No.: 19800013
As soon as the address and application fee have been received, we will
accept and confirm your FOI application, and will respond to you as soon
as possible, but within 45 days.
Please contact me again if any of the above is unclear or you have further
questions. I am happy to contacted directly on 9488 6255 or
[1][email address] if you do not wish to communicate
some of the above information via the Right to Know website.
Also, if you would like to know more about the FOI process you can go to
the Office of the Information Commissioner’s website
[2]http://foi.wa.gov.au/.
