Dear Department of Veterans' Affairs,

I’m writing this because the Helping Heroes contact has advised she was given a DVA referral and that my Rehab program has been approved, but I’ve not seen either of these two documents.

To assist me can you provide an FOI copy of:

(A) DVAs referral of my case to Helping Heroes.
(B) DVAs approval of my Rehab Return to Work

Yours faithfully,

Mary-Ann MARTINEK
