request. Please ensure to include the following information; • Date, time,
location and a description of the incident • If the incident involves
yourself and anyone else, please describe what you or other people were
wearing at the time Such a description can sometimes help the SMF identify
the relevant CCTV footage. Should you require access to any other
information please send an email request to
[RMS request email] and our staff will respond to your
enquiry on Monday, 9 January 2017.

Please note Transport for NSW office are closed for the Christmas period
and will resume business as usual on Monday 9 January 2017 and will
respond to your requests after this date

You are best to request this type of information by lodging an Access Application under the Government Information (Public Access) Act 2009 (GIPA Act) http://www.transport.nsw.gov.au/about/ac...

There is an application fee of $30, which you can pay by credit card, cheque or money order. Alternatively, you can make the payment at your local Motor Registry or Service NSW office, should you wish to lodge the application.
