Dear George Giannakopoulos, 

I am writing to acknowledge your request for documents under Freedom of
Information (FOI), received by the Aged Care Quality and Safety Commission
(Commission) on 27 November 2025.

The Commission’s decision on whether to grant access to the requested
documents is due on 27 December 2025. Please note that if we are required
to consult with third parties in relation to the documents this timeframe
may be extended by 30 days.

You requested:

“I kindly request all notes, emails, reports and any information in
relation to the complaint S25/029195”.

It would assist us if could you please respond to the following questions
within 2 business days: 

• Would you agree to duplicate documents, blank pages and standard
Commission fact sheets being removed from the scope of your request?
• Are you agreeable to excluding, from the scope of your request,
correspondence already in your possession, including any
communications you have sent to or received directly from the
Commission?
• Do you agree to the deletion of Commission staff names, positions and
contact details from any documents released to you under FOI?
• Do you hold any authorisation to receive the care recipient’s personal
information? If so, please provide a copy of the instrument of
authorisation. If not, please provide their contact details or those
of their representative so we can consult them.
• The documents may contain information in relation to which we need to
consult third parties, such as the consumer or approved provider:

name as the FOI applicant. Do you consent to the Commission naming
you as FOI applicant in any consultations we undertake?

Personal privacy
Dear George,

Thank you for your email, in order for the Commission to release any
personal information relating to your late father, please provide a copy
of the instrument of authorisation such as a executor of estate or court
order to show that you are authorised to receive information you have
requested.

It is because for the purposes of FOI, the Commission has a legal
requirement to satisfy itself that you are authorised to receive your late
father's information. Please note that generally all Powers of
Attorney,(as well as Guardianships and authorised signatories) cease once
the person dies.

Please also note that since you are using the right to know platform that
any documents that are released to you will be made publicly available.

My father had a Greek will and not an Australian one. I can share that with you, as well as the english translation listing myself and my brother Theodore as the main beneficiaries of his estate and for proof that we are his children if that would surfice?
