Dear NSW Department of Family and Community Services,

1. Could you please provide the number of applications each year from 2015-current that are applied for on the basis of a diagnosis of Myalgic Encephalomyelitis (ME) also known as Chronic fatigue Syndrome (CFS) or Chronic Fatigue.

2. Could you provide any inforomation that relates to policys, precedures, memo's, emails that contain information to lead the compaion card program to the vew that "me/cfs is not likley to meet the critera" for the companion card program as per the application form.
