Dear NT Police, Fire and Emergency Services,

I am an employee of DWF Adjusting (Australia) Pty Ltd (DWFAA). We have been appointed by Comcover to investigate the theft of a buggy at Wudicupildiyerr Ranger Station, Wudicupildiyerr NT 0822 on 6 December 2021.

Comcover have instructed us to obtain a copy of the relevant NT Police Incident Report.

Could you please advise the process I need to follow in order to obtain a copy of the Report and also the fee charged.

To retrieve a copy of stolen property report will need to go through the GPI Process, however it does need to be relevant to the person receiving the report (meaning they need to be involved to obtain the report). If so, please find the attached General Property Application Form to fill out. Please include the PROMIS Number, the Owner of the property stolen, date of incident, incident location. Please also supply us with any other relevant material, any rego numbers, other TP involved - Giving us more information helps us determine whether the report is releasable to the one applying. It is a fee of $62.00, which I will need an attached copy as proof. - Payment options can be seen on 3rd page.

Once completed send the form to the email listed on the application from.

I would like to add (just in case) that if you are a third party to the incident and were not originally involved, it will go through a different form. This will go through a Freedom of Information form as its retrieving third party information. I have also attached an FOI Application Form to fill out. Just fill out the description part explaining what you would like to obtain (PROMIS number) and attached a certified copy of ID - this can be done at any police station, JP, Court Clark, Pharmacist, NTPFES Employees, Justice of the peace or Commissioner of Oaths. An FOI is a fee of $30.00 which we will also need proof of.

Once completed send the form to the email listed on the application form.
