Dear L Gleeson,

Thank you for your email.

Access to Government held information needs to be applied for under the Government Information (Public Access) Act 2009 (GIPA Act). You can make an informal or formal application under the GIPA Act.

Informal application – there is no application fee, turnaround time is approximately 4 weeks and you have no review rights should you be aggrieved by our decision.

Formal application – there is a $30.00 application fee, legislative time frames apply and you have review rights.

I have attached the formal application form for your consideration.

Should you wish to make an informal request, please confirm this via return email (no application form is required).

If you have any queries please do not hesitate to contact me.

Kind Regards,

Stephen Wilson
