Dear Department of Health,

I write to you under the Freedom of Information Act 1982 pursuing;
Information/supporting documentation regarding seventy patients that were prescribed the incorrect dosage of chemotherapy over the course of three-years at St Vincent's Hospital, Sydney.
How does the Department of Health aim to avoid this issue recurring?
What procedures have been implemented?
Due to this incident have any patients incurred difficulties in recovery, if so what are these difficulties and how is the department/St Vincent's Hospital going to deal with the issue?

Review and recommendations in relation to an incident in a public hospital is a state/territory health government matter. As such the Commonwealth Department of Health does not hold this type of documents.
