Dear National Disability Insurance Agency,

By operation of section 15(5) of the FOI Act, FOI decisions must be issued within 30 days, unless an extension applies (as stated in section 15, 15AA and 15AC of the Act). According to my records, the due date for this decision was May 4.

Please advise whether a decision has been issued on this request, or if a deemed refusal is taken to have occurred by operation of section 15AC of the Act.

I note that the web site I am sending this message through has previously had technical issues receiving email from the NDIA, so I ask that you please consider resending any decision documents or other material in your reply, should they exist.

Web links are provided below my signature here, to enable you to contact this web site's technical team (or to request an appropriate person/group within the NDIA to do so), should you consider that reasonably required. They can attach your response if it is not delivered for whatever reason.

Yours faithfully,

Andrew Smith
