Dear Victorian Department of Health and Human Services,

I would like to request any documents containing fingerprints or a signature for Mehmet Demir with date of birth of 28/04/1967. Mehmet Ozdemir, my husband, passed away and sadly we have very minimal memories and would like to know if there are any fingerprints or signature on any files so that we can have some keepsake jewellery made up for our 10 year old daughter and sons.

Dear Victorian Department of Health and Human Services,

I would like to request any documents containing fingerprints or a
signature for Mehmet Demir with date of birth of 28/04/1967. Mehmet

Dear Ms Ozdemir

I refer to your application under the Freedom of Information Act 1982 (the
Act) regarding access to:

"any documents containing fingerprints or a signature for Mehmet Demir
with date of birth of 28/04/1967"

Your application was received on 22 October 2018. The application has been
waived in this instance.
I write to advise that your application is currently unclear and that we
need further information from you before we can process it.

We would appreciate you providing a response by 20 November 2018 to the
concerns outlined below. Your response can be provided to
[Victorian Department of Health and Human Services request email] or via reply to this email.

1. Clarification
Your application as currently worded does not comply with section 17(2) of
the Act because you have not provided enough information to identify
relevant documents. Relevant documents cannot be identified because:

* You have not specified a subject matter
* You have not specified a date range
* You have not specified the types of documents you seek

This means that your request may not proceed unless you can amend your
application so that it does comply with the Act and specific documents can
be identified.

To assist you with your clarification you may wish to provide alternative
wording that indicates:

* The type of documents you are requesting (for example, child
protection, wardship, disability, psychiatric, housing, employee)
* The area of the department you would like us to search for documents
* Whether you are requesting documents from the Victorian Department of
Health and Human Services or another agency (for example the Federal
Department of Human Services who are responsible for Medicare and
Centrelink)

We cannot proceed with your application until we receive a written
response from you. If we have not received a response from you by the due
date above, I will assume that you do not wish to clarify your application
and no further action will be taken by the department.

You may offer different wording to what we have suggested however, if this
does not remove the grounds for refusal outlined in this letter, the
application may not proceed.
Please note, even with clarification, it may be determined that the work
involved in processing your request would divert the resources of the
department substantially and unreasonably from its other operations and
further consultation may be necessary. If this is the case, you will be
advised in writing.

2. Certified Identification and Linking Documents
Processing a freedom of information request requires a number of steps,
including searching for, identifying, and assessing the requested
documents. Before we can process your request, we need to sufficiently
establish your identity and right of access to records. To do so we ask
that you provide us with:

* a certified copy of your identification (Please see attached fact
sheet on how to have your identification certified);
* Documents linking you to the person whose documents you are seeking;
* A death certificate of the person whose documents you are seeking.
