Thank you for your enquiry regarding Sunshine Coast Health & Hospital Services records.

All medical records are held by the individual public hospitals/health facilities which come under the jurisdiction of their relevant Hospital and Health Service (HHS). In order to access these records you should contact the relevant HHS directly. Each of the HHSs have a delegated Right to Information (RTI) officer who can assist you with your request. As the records you are seeking to access are not your own personal records, you will need to complete and submit a Right to Information and Information Privacy Access Application form (attached) and send to the relevant public hospital/facility, along with a mandatory application fee: http://www.rti.qld.gov.au/fees-and-charges

You should provide as much information as you can in order to assist with searches for the requested records. You will also be required to provide original certified copies of your evidence of identity (eg. drivers license/birth certificate/passport).

I would suggest that you contact the relevant HHS before submitting your application to ascertain whether or not they would still hold these records and they can assist you with any other enquiries you may have regarding making an application and the payment of your application fee. Please note that the HHSs are separate entities, and as such you will need to submit a separate application, plus application fee, to each HHS if there are more than one.

Also, the Department of Health only has jurisdiction over public hospitals. Should the records be held by a private hospital, GP or private specialist, you will need to contact them directly.
