Dear Department of Defence,

I am requesting the Quick Assessment that was referred to ADFIS while I was still employed with 23 Field Regiment until the 11th of November 2011. Apparently this QA was conducted while I was there working and finalised on the 7th November 2011. I am requesting this document because I was unaware of this QA and it was not shown to me before I resigned.
