I would like to request a list of:

1. Approved pharmacies, for postcode 4000,4178,4119, 4506 that have been registered with QLD Health as of 30th June 2017.

2. Associated PBS Approval number for Approved pharmacies, for postcode 4000,4178,4119, 4506 that have been registered with QLD Health as of 30th June 2017.

3. If the Approved pharmacies, for postcode 4000,4178,4119, 4506, have indicated if they are Non-PBS pharmacy of 30th June 2017.

This information is on Section 1, Page 2 of Pharmacy Change of Ownership Notification Form that is lodged with QLD Health
