Childcare Centres in NSW Proof of Vaccination Forms
Dear Advisory Committee on the Safety of Vaccines,
This is a request made under the Freedom Of Information Act
I am seeking access to any feedback and complaints regarding child care centres requesting proof of vaccination forms in NSW. This could be from either the Childcare Centre staff members or parents who are wanting to enrol their kids. I would appreciate access to the years 2010-2015.

The Commonwealth Department of Health does not hold information regarding any feedback or complaints with respect to child care centres in NSW requesting proof of vaccination.
You may wish to direct your request to the relevant state/territory health department. Details about how to request documents from state/territory health department can, generally, be found on their website.
The Commonwealth Department of Health will consider your request withdrawn and no further action will be taken in relation to your request.
