I am seeking access to any correspondence (whether it be feedback or complaints) between Health officials, Child Care workers, and or Parents in regards to Child Care Centres requesting proof of vaccination forms in NSW from 2012 - 2016.

The Commonwealth Department of Health does not hold information regarding any feedback or complaints from health officials, child care workers, or parents in regards to child care centres requesting proof of vaccination forms in NSW from 2012 - 2016.
You may wish to direct your request to the relevant state health department. Details about how to request documents from state health department can, generally, be found on their website.
The Commonwealth Department of Health will consider your request withdrawn and no further action will be taken in relation to your request.
